We are seeking a skilled Nurse Practitioner or Physician Assistant to join our team as a Health Risk Assessment Professional. This role involves conducting comprehensive health assessments for older adults and other at-risk individuals in their homes, skilled facilities, or via telehealth.
About the RoleThis is a PRN/Part Time position with flexible scheduling, offering competitive pay of $110 per completed assessment. The ideal candidate will have a strong background in healthcare, excellent communication skills, and the ability to work independently.
Responsibilities- Conduct adult and geriatric assessments, medical history, diagnosis, and treatment
- Collaborate with physicians, case managers, social workers, and other healthcare professionals
- Provide education and support to patients and their families
- Travel to patient locations, including homes and skilled facilities
- Master's degree in nursing or physician assistant studies
- Current RN and NP or PA licensure, with prescription authority
- Board certification in a Matrix-approved specialty
- 1 year of experience as a Nurse Practitioner or Physician Assistant
- Strong computer skills and familiarity with EMR software
- Competitive hourly rate of $110
