Nursing Care Coordinator

American Health Partners is seeking a skilled Licensed Practical Nurse to join our team as a Nursing Care Coordinator. In this role, you will play a critical part in ensuring the highest quality of care for our patients.
About the RoleThe successful candidate will be responsible for assessing patient health needs, developing and implementing nursing care plans, and maintaining accurate medical records under the direction of the Director and Assistant Director of Nursing Services.
Key Responsibilities- Receive delegated orders from the Director of Nursing and ensure their execution by staff members.
- Supervise and direct medication nurses and nursing aides in their daily tasks.
- Oversee all resident/patient treatments, including monitoring intake and output records.
- Assist physicians and the Director of Nursing in patient treatment and examination.
- Document nurses' notes, physicians' orders, discharge plans, dietary changes, reports, evaluations, and studies in a timely manner.
To be considered for this position, you must have a strong foundation in nursing principles and practices, excellent communication skills, and the ability to work effectively in a fast-paced environment.
