Foster Care Coordinator

We are seeking a highly skilled and compassionate Foster Care Coordinator to join our team at UnitedHealthcare. As a Foster Care Coordinator, you will play a critical role in providing trauma-informed care to children and youth in foster care, ensuring they receive the support and services they need to thrive.
Key Responsibilities- Assess and develop individualized care plans for children and youth in foster care, taking into account their unique needs and circumstances.
- Coordinate and monitor referrals for psychosocial, behavioral, and medical services, ensuring seamless care transitions.
- Communicate effectively with stakeholders, including caregivers, social workers, and healthcare providers, to ensure quality coordinated care.
- Advocate for children and youth, ensuring their needs and choices are represented and supported by the healthcare team.
- Provide education and support to caregivers and children, promoting healthy relationships and outcomes.
- Master's degree in social work, nursing, psychology, or related field.
- 2+ years of experience working in community health, preferably with children and youth in foster care.
- Strong communication and interpersonal skills, with ability to work effectively with diverse stakeholders.
- Ability to travel within assigned region to visit members in foster homes and other settings.
- Licensed Social Worker or clinical degree.
- Experience working with complex medical or behavioral needs.
- Background in managing populations with diverse needs.
