About the Role

We are seeking a dedicated and compassionate Hospice Nurse to join our team in the West Portland area.

Job Summary

This is an excellent opportunity for a skilled nurse to provide high-quality care to patients with life-limiting illnesses. As a Hospice Nurse, you will work closely with our interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each patient.

Key Responsibilities
  • Provide high-quality, person-centered care to patients with life-limiting illnesses.
  • Develop and implement individualized care plans that meet the unique needs of each patient.
  • Collaborate with our interdisciplinary team to ensure seamless care coordination.
  • Uphold the standards of confidentiality, attendance, and punctuality.
Requirements

To be considered for this role, you must have:

  • Education to meet certification, license, or registration requirements.
  • Oregon Registered Nurse License upon hire.
  • At least 2 years of medical/surgical experience in an acute care setting, or a combination of medical/surgical, long-term care, or home health/hospice experience.
  • Experience using electronic medical record software.
