Pain Management Clinical Pharmacist Job Description

At Potomac Healthcare Solutions, we are seeking a highly skilled Clinical Pharmacist to join our team in a Pain Management role. The ideal candidate will have a strong background in clinical pharmacy practice, with a focus on pain management.

Key Responsibilities:
  • Deliver comprehensive pharmaceutical services to patients with pain management needs.
  • Work closely with clinical staff to develop patient wellness programs, including smoking cessation and asthma classes.
  • Provide medication information to clinical staff, including in-service training on new medications and appropriate use.
  • Provide clinical direction to corpsmen and pharmacy technicians, and participate in job performance evaluations.
  • Maintain inventory control of pharmaceuticals, including scheduled controlled substances, and ensure pharmacy security.
  • Utilize pharmacy computer and robotic systems in accordance with MTF protocols.
  • Participate in a multidisciplinary team to provide consultative services for patients with pain management needs.
  • Work with physicians to provide drug therapy recommendations, perform polypharmacy management, and provide patient education.
  • Conduct medication reviews and reconciliations for chronic pain patients, and develop strategies for patients with histories of non-compliance, abuse, or diversion.
  • Document and communicate all information as appropriate, including pain patient agreements and pharmacotherapeutic treatment findings and recommendations.
  • Provide education to patients and their families/caretakers on medications and non-drug therapy, and provide compliance aids such as medication charts and pill boxes.
  • Extend existing prescriptions as appropriate, and ensure patients receive follow-up appointments with the appropriate individual(s).
  • Order consults and referrals as necessary.

Requirements:
  • Doctor of Pharmacy (Pharm.D.) degree from an accredited institution.
  • Completion of a clinical pharmacy residency or fellowship, or documentation of appropriate education, training, and/or CME in clinical pharmacy practice.
  • Possession of a graduate certificate from an accredited pharmacy residency program, or a minimum of 12 months of experience within the preceding 18 months as a Clinical Pharmacist in an ambulatory care pharmacy setting.
  • Current, full, active, and unrestricted license to practice as a pharmacist.
  • Must be a US Citizen.
