Job Overview

Soliant Health is seeking a Pharmacist to work in a Long-Term Acute Care (LTAC) hospital. The ideal candidate will have a Kentucky Pharmacist license and some prior exposure to inpatient pharmacy practice.

Key Responsibilities
  • Review and verify medication orders for accuracy and appropriateness.
  • Oversee pharmacy technicians to ensure the safe and efficient preparation and dispensing of medications.
  • Provide consultation services to physicians and nurses regarding drug therapy, dosing, and medication management.
  • Conduct pharmacokinetic monitoring and dosing adjustments for patients as needed.
  • Participate in patient care by recommending appropriate therapy modifications, reviewing patient charts, and monitoring drug regimens to ensure optimal outcomes.
Ideal Candidate Qualifications
  • Must hold an active and unrestricted Kentucky pharmacist license.
  • Exposure to inpatient pharmacy practice is preferred. Experience working in an LTAC or hospital setting is a plus.
  • Strong knowledge of pharmacokinetics, medication safety protocols, and clinical consultation practices.
  • Familiarity with electronic medical records (EMR) systems is an advantage.
