Job Summary

As a Licensed Practical Nurse on our Long-Term Care team, you will play a vital role in providing high-quality patient care to our residents. Under the supervision of the Registered Nurse/Director of Nursing, you will develop and implement individualized nursing care plans to meet the unique needs of each resident.

Responsibilities
  • Provide direct patient care, including administering medications, performing vital signs, and assisting with daily living activities
  • Conduct thorough assessments of residents' physical and emotional needs, and develop plans to address these needs
  • Collaborate with the interdisciplinary team to ensure seamless care coordination and communication
  • Participate in unit-level decision-making and problem-solving to improve patient outcomes and satisfaction
Requirements
  • Graduate of an accredited school of nursing with a current LPN license
  • Basic computer skills and proficiency in electronic medical records
  • Ability to work effectively in a fast-paced, dynamic environment
  • Strong communication and interpersonal skills, with a focus on empathy and compassion
