Job Description

St. Anne Institute is seeking a skilled Clinical Therapist to join our team in Hudson Falls, NY.

Job Summary: We are looking for a dynamic, trauma-informed Clinical Therapist to provide individual, family, and group therapy to assigned clients. The ideal candidate will have experience working with individuals, families, and groups, and a strong commitment to trauma-informed care.

Responsibilities:

  • Develop and implement comprehensive clinical assessments and treatment plans for assigned clients.
  • Provide individual, group, and family-focused therapy, including outreach and advocacy.
  • Serve as a liaison with community agencies and referral sources.
  • Coordinate and complete necessary reports and correspondence.
  • Attend weekly individual and team/group supervision.
  • Facilitate case planning for each client/family.
  • Work cooperatively with client's case manager and/or outside service providers to coordinate clinical services and resolve issues.
  • Maintain updated assessments and progress notes, and record updates in the NYS OCFS CONNECTIONS electronic case management system and/or in the SAI Electronic Health Record as required by contract or regulation.
  • Present clinical treatment summaries on clients at weekly clinical team meetings.
  • Attend all clinical department staff meetings and supervisory meetings, as required.
  • Stay current with trends in clinical treatment, trauma-informed care, and evidence-based models through reading, in-service training, and conferences.

Requirements:

  • LMSW, LCSW, LMHC, or other relevant degree and experience working with individuals, families, and groups.
  • Ability to function as part of a multi-disciplinary treatment team.
  • Excellent communication and written skills.
  • Experience with electronic health records, CFTSS, and NYS OCFS CONNECTIONS is preferred.
