Overview

As a skilled and compassionate nurse, you will play a vital role in delivering high-quality care to our clients in the comfort of their own homes.

Key Responsibilities
  1. Assessment and Planning: Conduct comprehensive assessments of clients' physical and functional needs, as well as their psychosocial status and needs, home safety, and family caregiver burden.
  2. Care Coordination: Collaborate with interdisciplinary team members to develop and implement comprehensive care plans, ensuring seamless coordination of services.
  3. Direct Care: Provide direct care to clients, including personal care, nursing rehabilitation tasks, and management of medications and other healthcare needs.
  4. Documentation and Communication: Maintain accurate and detailed records of client care, communicate effectively with clients, families, and healthcare providers, and participate in regular team meetings to ensure continuity of care.
Requirements
  • Current NY State Registered Nurse License
  • Valid Driver's License and Reliable Transportation for Home Visits
  • Associate Degree (BSN Preferred)
Work Environment

This position requires frequent travel to clients' homes and/or other facilities to deliver direct care, and may involve working in a fast-paced, dynamic environment with multiple priorities and deadlines.
