Flexible Nurse Practitioner

Our clinicians break through traditional barriers to care by meeting those members where they are, helping older adults and other at-risk individuals enjoy a better quality of care, experience improved health outcomes, and identify chronic conditions that may otherwise go undiagnosed.
Job DescriptionAs a Nurse Practitioner - PRN/Part Time, you will conduct adult/geriatric assessments, medical history, diagnosis, and treatment, as well as provide health education, physician referrals, case management referrals, follow-up, and clear documentation according to Matrix guidelines and protocols.
You will work collaboratively with physicians, case managers, social workers, family members, key caregivers, and any appropriate ancillary medical personnel as needed.
Your responsibilities will also include collaborating with Primary Care Physicians (PCPs) on patient education and providing follow-up, as well as providing services in various venues, including home visits, skilled facility visits, and telehealth visits.
Key Qualifications- Master's degree required or commensurate experience and satisfactory completion of NP licensure
- Current RN and NP licensure in the state of practice, including prescription authority or the ability to obtain prescriptive authority
- Board certification by the AANP, ANCC, or AACCN in a Matrix-approved specialty
- Current BLS, ACLS, or CPR certification
- Minimum 1 year of experience as a Nurse Practitioner
- Strong computer skills and familiarity with employee health/medical record software
- Excellent verbal and written communication skills with patients, clients, and colleagues
This opportunity offers a competitive compensation package, including an estimated salary of $90.00 per completed assessment, as well as a mileage reimbursement plan.
PRN/Part-Time providers are non-benefited except for 401(k) and limited voluntary benefits.
This is a flexible scheduling opportunity with no on-call hours, evening shifts, or weekend hours unless desired.
