Job Summary

We seek a highly organized and detail-oriented Behavioral Health Advocate to join our team. As a key member of our staff, you will provide care coordination, case management, and mental health intervention services to individuals and families experiencing severe and persistent mental illness.

About the Role

The ideal candidate will possess excellent clinical, administrative, and communication skills, with the ability to work collaboratively with clients, family members, and healthcare providers. You will be responsible for assessing client needs, developing and implementing treatment plans, providing behavioral health interventions, and connecting clients with community resources.

Responsibilities
  • Provide care coordination, case management, and mental health intervention services to clients and families experiencing severe and persistent mental illness.
  • Assess client needs and develop individualized treatment plans to address their specific needs and goals.
  • Provide behavioral health interventions, including education, support, and counseling services.
  • Connect clients with community resources, such as housing, employment, and educational services.
  • Collaborate with healthcare providers, family members, and other stakeholders to ensure comprehensive care and support.
  • Maintain accurate and confidential records of client interactions and progress.
Requirements
  • High School Diploma or equivalent required; Bachelor's degree in a related field preferred.
  • Certification as a Certified Alcohol and Drug Counselor (CDCA) or similar certification preferred.
  • Excellent communication, organizational, and problem-solving skills.
  • Ability to work independently and collaboratively as part of a team.
  • BLS certification within 90 days of hire.
