Palliative Care Social Worker

Avera Medical Group Palliative Medicine Yankton seeks a skilled PRN Social Worker to join our multidisciplinary team. As a key component of our clinical care team, you will provide social work services to patients and families in the clinic setting, addressing medically related emotional and social needs on an individual basis.
Key Responsibilities:
- Assess the needs of patients facing chronic illness and facilitate care plans with patient/family to promote effective care.
- Maintain a current knowledge base of available community agencies and resources to facilitate appropriate referrals.
- Provide direct therapeutic services to patients as part of a comprehensive patient care plan.
Requirements:
- Master's degree in Social Work from an accredited program required.
- Licensure as a Social Worker required. Qualified Mental Health Professional Credential preferred.
Work Schedule:
PRN as needed.
Compensation:
The hourly range for this position is $28.75 - $43.50 per hour, dependent upon experience.
