Job Description

The Geriatric Nurse Specialist will be responsible for providing top-notch medical care to our residents, ensuring their physical, emotional, and social well-being. This includes conducting regular wellness visits, monitoring vital signs, and collaborating with healthcare professionals to develop personalized care plans.

Responsibilities:
  • Conduct thorough assessments of residents' health needs and provide tailored care solutions.
  • Maintain accurate records of resident care, including weights, vital signs, and medication administration.
  • Collaborate with Resident Care Directors and other healthcare professionals to ensure seamless care delivery.
  • Develop and implement infection control practices to maintain a safe environment for residents and staff.
  • Participate in quality assurance meetings to identify areas for improvement and contribute to enhancing resident care.
Requirements and Qualifications

To be successful as a Geriatric Nurse Specialist, you must possess:

  • A current state license as a Licensed Practical Nurse/Licensed Vocational Nurse (LPN/LVN).
  • Minimum of one year experience in home health, assisted living, hospital, or long-term care environment.
  • Demonstrated knowledge of nursing practices, techniques, and methods applied to health and wellness resident care.
  • Excellent communication and interpersonal skills, with the ability to work effectively with residents, families, and healthcare teams.
