We are seeking a dedicated professional to join our team as a Case Manager, supporting our residents in achieving housing stability and self-sufficiency.

The ideal candidate will have a strong understanding of the challenges faced by individuals and families in at-risk communities and be passionate about helping them build better lives.

Key Responsibilities:

  • Provide supportive case management services to resident households, including goal formulation and individualized action plans
  • Conduct assessments and periodic reassessments on progress toward goals
  • Provide service referrals and coordination of service delivery
  • Administer direct assistance and financial support for residents and their children
  • Participate in planning and implementing program activities and community events

Requirements:

  • Bachelor's degree in social work, psychology, counseling, sociology, or related field
  • Strong analytical and problem-solving skills
  • Excellent communication and interpersonal skills
  • Cultural competency and ability to work with diverse communities
