Xpress Wellness Urgent Care, a reputable healthcare provider in the long-term care facility sector, is seeking a skilled Advanced Practice Provider to join their dedicated team.
Job OverviewThe ideal candidate will provide high-quality care to elderly and medically complex residents, working collaboratively with physicians, nursing staff, and other healthcare professionals to ensure comprehensive care and manage chronic conditions.
Responsibilities- Conduct regular rounds in the facility to gather patient history, perform examinations, and document findings in electronic medical records.
- Develop and maintain a rounding schedule, notes, lab reports, and imaging results in the EHR for patients seen within the practice.
- Communicate effectively with the Director of Nursing or floor nurses before and after rounds to obtain their concerns and relay new orders, ensuring timely activation of these orders.
- Serve as the primary point of contact for nurses regarding patients seen, necessitating occasional after-hours communication via phone or text.
- Maintain accurate and detailed patient charts, including follow-up notes and treatment courses.
- Assume additional responsibilities and projects as assigned, considering this position's safety-sensitive nature.
- Active Nurse Practitioner or Physician Assistant license
- Board certification in Nurse Practitioner or Physician Assistant
- Ongoing state and federal licensure requirements, including DEA registration
- Current BLS certification
- Demonstrated knowledge of relevant state and federal regulations, including OSHA, HIPAA, blood-borne pathogens, and others
- Exceptional communication and interpersonal skills
- Ability to thrive in a fast-paced, high-stress environment while maintaining efficiency and effectiveness
- Familiarity with electronic medical record systems
- Understanding of medical coding and billing principles
- Strong decision-making and problem-solving skills
- Capacity to collaborate effectively with other healthcare professionals and staff members
- Proficiency in common PC applications, including Internet, email, and Microsoft Office
