Resident Care Coordinator Job Description

TerraBella Senior Living, a leading provider of senior living and care options in the Carolinas, Virginia, Kentucky, Georgia, and Tennessee, is seeking an experienced Resident Care Coordinator to join our team at Deane Hill Place.

Job Summary:

We are looking for a compassionate and organized individual to coordinate resident care needs, ensuring seamless delivery of services while maintaining compliance with company policies and state regulations. The successful candidate will possess excellent communication skills, a warm demeanor, and a genuine interest in working with seniors.

Key Responsibilities:
  • Coordinate Resident Care Needs: Develop and implement effective care plans, manage scheduling, and maintain accurate records to ensure timely and efficient delivery of services.
  • Maintain Accurate Records: Ensure all documents related to resident care are properly filed, updated, and verified by physicians as necessary.
  • Medication Administration: Oversee daily medication administration processes, conduct cart audits, and intervene as needed to provide support or training.
  • Care Staff Training and Supervision: Assist Director of Health & Wellness in training new staff members, including Care Managers and Medication Technicians, and provide supervision in their absence.
  • Scheduling and Transportation: Arrange appointments and transportation for residents, ensuring compliance with physician visit logs and procedures.
  • Fire Safety and Disaster Plan: Understand and implement evacuation procedures during drills and actual emergencies.
  • Perform Other Duties: Assist with data collection, reporting processes, and perform other duties assigned by the Director of Health & Wellness.
Requirements:
  • A licensed practical nurse (LPN) in the state of Tennessee or a licensed Medication Technician certified in Tennessee, CNA with at least one year of experience preferred.
  • Excellent written and verbal communication skills.
  • Interest in working with seniors.
  • Demonstrated compassion, warmth, and strong leadership skills.
  • Knowledge of community agencies and appreciation for social and life history status of residents.
