Job Summary

We are seeking a skilled Community Health Worker to join our Case Management team at Ascension. As a key member of our team, you will play a vital role in providing exceptional patient care and support.

Key Responsibilities
  • Perform initial health screenings for newly enrolled members and convey the purpose and services of our case management programs.
  • Provide basic health information, collect data, and perform clerical duties to support RN Case Managers and Social Workers.
  • Work collaboratively with the healthcare team to achieve case management goals, care transitions, and health literacy.
  • Support members in accessing health and social services, including finding primary care physicians and scheduling appointments.
  • Utilize electronic resources and registries to collect, report, and document data for the case management process.
  • Assist with monitoring preventive health care screenings and open care gaps, and conduct outreach to link members back to their medical home.
  • Serve as a point of contact for members receiving case management and social work services.
  • Perform non-clinical case management functions, including outreach, social support, resource linkage, interpretation/translation, and transportation.
  • May act as a preceptor for newly hired case management coordinators and participate in quality improvement activities.
Requirements
  • High School diploma equivalency or 1 year of applicable cumulative job-specific experience required.
  • Additional preferences include 2+ years of experience in patient care or public health settings, 1+ year of case management experience, and certification as a Community Health Worker or Medical Assistant.
