We are seeking a skilled and compassionate certified Nurse Practitioner to join our provider network at Advantmed. In this role, you will be responsible for conducting in-home wellness risk adjustment assessments for Medicare members and other at-risk populations.

Our goal is to provide high-quality care to our members while ensuring accurate and comprehensive risk adjustment coding. This leads to greater value-based care and improved patient outcomes.

You will play a pivotal role in assessing the overall health and well-being of member beneficiaries through comprehensive evaluations, including physical examinations, vital sign reviews, medication history checks, and social assessments.

Your primary objective will be to deliver individualized quality care to the elderly population in their homes, building relationships of trust through exceptional communication and empathy.

You will also assist in closing quality care gaps by identifying screenings and lab needs, and providing patient health education on topics such as pain management and medication adherence.

This 1099 (PRN) role offers tremendous flexibility and opportunity for those balancing competing priorities. You will have access to state-of-the-art technology, dedicated coordinator support, and advanced member scheduling coverage.

The estimated salary for this position is $13,400 per month, based on a minimum of 30 hours worked per month.

Requirements:

  • A valid unencumbered NP License for the state you will be working in
  • Previous in-home risk assessment experience preferred
  • Previous 1099 (PRN) experience is preferred but not required
  • 3 years patient care experience preferred (primary care/adult/geriatric, EMR)
