Job Responsibilities
This role involves providing competent clinical nursing care to patients in their homes, utilizing the nursing process to assess, plan, implement, and evaluate patient care. The ideal candidate will possess excellent communication and interpersonal skills, with the ability to work collaboratively with patients, families, and interdisciplinary teams to achieve optimal outcomes. Key responsibilities include:
•Assessment and Planning: Conduct comprehensive assessments of patients' physical, emotional, and social needs, developing personalized care plans that address their unique requirements and preferences.
•Implementation and Evaluation: Deliver evidence-based care, monitoring patients' progress, and adjusting interventions as needed to ensure seamless transitions across the continuum of care.
•Patient Education: Provide education and support to patients and their families, empowering them to manage their conditions effectively and make informed decisions about their healthcare.
•Collaboration and Communication: Foster strong relationships with patients, families, and colleagues, promoting open communication, empathy, and understanding throughout the care journey.
Requirements and Qualifications
To succeed in this role, you must be a registered nurse licensed in North Carolina, with at least one year of experience caring for acute and chronically ill patients. You should possess excellent critical thinking, problem-solving, and time management skills, with the ability to adapt to changing circumstances and priorities. A bachelor's degree in nursing or a related field is preferred.
Salary and Benefits
We offer a competitive salary range of $31.52 - $44.56 per hour, commensurate with your level of experience.
