About the JobAn Infusion Nurse II plays a critical role in our mission by coordinating and providing intermediate direct patient care to patients in their homes or alternative infusion suites. This position involves ensuring patient safety while maintaining continuity and compliance under a physician's plan of care. As an experienced nurse, you will have the opportunity to initiate, develop, and implement intermediate nursing plans of care treatments, evaluating patient progress towards established goals.
You will assess patient needs and physical status at each skilled visit through health data access and patient interviews. Re-evaluating patient needs through physical reassessment, response to therapy, and supplemental physician orders is essential. Effective communication with our clinical team, medical providers, patients, and families/caregivers is vital for facilitating continuity of care. You will prepare clinical documentation in real-time during visits and submit it to the nursing department within established timelines to ensure billing optimization.
This role requires a high level of professionalism, as you will be responsible for demonstrating compliance with agency operations, policies, and procedures, as well as professional standards, local, state, federal regulations/guidelines, and accreditation standards. Your ability to maintain accurate records, respond promptly to patient requests, and initiate emergency procedures as necessary is crucial. You will also participate in multidisciplinary team conferences and provide precepting, training, and mentoring to other nurses for orientation and onboarding activities as assigned.
Requirements
- Active and unrestricted RN license in the state of practice.
- Minimum of 1 year of nursing experience.
- Current CPR certification required.
- Bachelor of Science in Nursing from an accredited school of nursing preferred.
- 1-2 years of previous infusion nursing experience preferred.
