Clinical Care Coordinator

Job Summary: We are seeking a Registered Nurse to join our clinical team at Genesis HealthCare. As a key member of our multidisciplinary team, you will play a vital role in providing high-quality patient care and collaborating with other healthcare professionals to develop effective care plans.

The ideal candidate will have a strong background in nursing assessments, medication administration, and collaborative care. Previous experience working in a related setting is preferred, but not required. CPR certification is also essential.
Responsibilities:- Conduct thorough nursing assessments and develop individualized care plans
- Collaborate with the nursing team and other disciplines to ensure seamless care delivery
- Administer medications and perform treatments as ordered by physicians
- Delegate tasks and supervise staff to ensure quality care delivery
- Communicate effectively with patients, families, and colleagues
To be successful in this role, you must possess:
- A graduate degree in nursing from an accredited institution
- Current licensure by the State Board of Nursing
- CPR certification
- One to three years of experience working as a RN in a related setting (preferred)
