OptumCare's Advanced Practice Provider Opportunity in Geriatric Primary Care

About the Role:

We are seeking an experienced Advanced Practice Provider to join our team as a Geriatric Homecare Physician. In this role, you will have the opportunity to deliver personalized care to elderly patients in the comfort of their own homes.

Key Responsibilities:

• Provide high-quality primary care to frail and at-risk elderly patients in their homes
• Have ownership of your patient panel and take pride in delivering exceptional patient outcomes
• Care for an average of six to eight patients daily, allowing for meaningful relationships and tailored care plans
• Provide coverage for Metrowest communities, serving a diverse population with unique needs

Required Qualifications:

• National Board Certification as an Advanced Practice Provider (e.g., NP or PA)
• Licensed by the Commonwealth of Massachusetts board of registration in Nursing

Preferred Qualifications:

• Minimum two years of experience as an APC in geriatric care
• Experience working in homecare settings, with a focus on delivering high-quality care in non-traditional settings

Pursue Your Career Goals:

$120,000 - $180,000 per year, based on experience and qualifications, plus benefits and bonuses.
