Job Description

We are seeking an Advanced Practice Provider (APP) to join our dynamic team in Connecticut. As an APP, you will be responsible for delivering high-quality medical care services to assigned members, managing both medical and behavioral conditions effectively, and collaborating with physicians and specialty providers as needed.

Your primary responsibilities will include:

  • Delivering cost-effective, quality care to assigned members
  • Managing medical and behavioral conditions effectively, in collaboration with physicians and specialty providers
  • Performing comprehensive assessments and documenting findings in compliance with documentation requirements and CMS regulations
  • Ensuring accurate ICD10 coding and documentation of diagnoses
  • Facilitating agreement and implementation of the member's plan of care by engaging facility staff, families, primary and specialty care physicians
Required Skills and Qualifications

To succeed in this role, you will need:

  • Active national NP or PA certification or the ability to obtain national certification in state of assignment by start date
  • For NPs: Active ANCC or AANP national certification in Family, Geriatrics, Adult, Adult-Gerontology Primary Care, or Adult-Gerontology Acute Care certification OR for PAs - Physician Assistant national certification through NCCPA
  • Active and unrestricted APC license in the State of Connecticut
  • Current active DEA licensure/prescriptive authority or ability to obtain post-hire, per state regulations (unless prohibited in state of practice)
  • Ability to lift a 30-pound bag in and out of car and to navigate stairs and a variety of dwelling conditions and configurations
  • Availability to work 24 hours per month, with expectations that 16 of the 24 hours/month could be during off-hours (after 5 pm, on weekends, and/or holidays) not to exceed 960 hours in a calendar year
Salary and Benefits

The hourly range for this role is $42.31 to $83.27 per hour.
