As a Community Health Worker with CAMBA's Initiating Change Around Reproductive Empowerment (iCARE) program, you will play a vital role in addressing maternal and infant health disparities in underserved communities. Your primary responsibility will be to conduct outreach and engage with high-need women and infants, providing them with access to essential services and resources.
Key Responsibilities:
- Develop and implement effective outreach strategies to identify and engage with target populations.
- Provide educational presentations and workshops on reproductive health, parenting, and community resources.
- Conduct initial intake and assessments, and create personalized service plans for participants.
- Facilitate referrals to healthcare providers, social services, and other community resources.
- Monitor participant progress, document outcomes, and adjust service plans as needed.
Requirements:
- High school diploma or equivalent, and 4 years of relevant experience.
- Excellent communication and interpersonal skills.
- Ability to work in a fast-paced environment and adapt to changing priorities.
