Nurse Practitioner

AleraCare is seeking a skilled and compassionate Part-Time Infusion Nurse Practitioner to join our team in Berkeley, CA. The ideal candidate will have a strong background in infusion nursing and excellent communication skills.
Key Responsibilities- Provide high-quality patient care and develop individualized plans of care in accordance with the patient's needs.
- Collaborate with the healthcare team to ensure seamless care coordination and effective communication.
- Conduct thorough patient evaluations and assessments, and document findings in a timely and accurate manner.
- Develop and implement educational programs to promote patient understanding and empowerment.
- Participate in performance improvement initiatives and contribute to the development of policies and procedures.
- Master's degree from an accredited Nurse Practitioner program.
- Current NP state license in the state of practice.
- At least 4-5 years of advanced practice registered nursing (APRN) experience, with a focus on ambulatory and infusion nursing.
- Strong interpersonal, time management, and organizational skills.
- Excellent written and verbal communication skills.
- Experience with electronic medical records (EMR) and ability to maintain accurate patient care records.
The Part-Time Infusion Nurse Practitioner will work a regular schedule, Wednesday through Friday, 8:00am-5:00pm. The ideal candidate will be able to work independently and as part of a team, with a strong focus on patient-centered care.
