Job SummaryWe are seeking a skilled Registered Nurse (RN) to join our team as a Home Health Part-Time professional. As a RN Home Health Part-Time, you will provide admission, case management, and follow-up skilled nursing visits for home health patients.
You will work closely with physicians to develop initial plans of treatment based on physician orders and patient assessments. Your hands-on care, management, and evaluation of the care plan will ensure that patients receive the best possible care.
You will also coordinate with other healthcare professionals, including Physical Therapists, Occupational Therapists, Home Health Aides, and external providers, to deliver comprehensive care to our patients.
Responsibilities- Provide admission, case management, and follow-up skilled nursing visits for home health patients.
- Develop initial plans of treatment in consultation with physicians and patient assessments.
- Implement patient care plans in conjunction with patients and families to achieve optimal resolution of needs/problems.
- Coordinate/oversee/supervise the work of Home Health Aides, Certified Home Health Aides, and Personal Care Workers.
- Prepare and complete needed clinical documentation, including visit/shift reports, updates/summarizes patient records, and confers with other healthcare disciplines.
To be successful in this role, you should have:
- Diploma, Associate, or Bachelor's Degree in Nursing
- Minimum of one year nursing experience preferred
- Strong med surg, ICU, ER, acute experience
- Home Health experience a plus
- Current and unrestricted Registered Nurse licensure
- Current CPR certification
- Valid driver's license, auto insurance, and reliable transportation
The estimated annual salary for this position is $74,400 - $98,800, reflecting a good faith estimate of starting base pay for full-time employment at the time of posting.
