Sunrise Senior Living is a leader in senior care, and we're seeking a compassionate Care Manager to join our team. As a Care Manager at Sunrise, you'll play a vital role in providing exceptional care and services to our residents.

You'll work closely with residents, their families, and our interdisciplinary team to create personalized care plans that cater to their unique needs and preferences.

Key Responsibilities- Deliver individualized care and support to a consistent group of residents, including assistance with activities of daily living and promoting independence.
- Develop and implement Individualized Service Plans (ISPs) that meet the unique needs and goals of each resident.
- Work collaboratively with the community team to ensure compliance with OSHA requirements and promote risk management practices.
- Participate in ongoing education and training to stay up-to-date on the latest developments in senior care and dementia management.
- A CNA or HHA license in good standing (certifications may vary by state).
- Demonstrated experience working with seniors and their families, preferably in an assisted living or memory care setting.
- Excellent communication and interpersonal skills, with the ability to build strong relationships with residents, families, and colleagues.
- Able to work independently and as part of a team, with a commitment to maintaining a safe and healthy work environment.
We're proud to offer a competitive salary range of $43,000 - $52,000 per year, based on location, experience, and qualifications. In addition to your base compensation, you may also be eligible for discretionary bonuses, tuition reimbursement, and opportunities for career growth and advancement within our organization.
