We are seeking a highly skilled and experienced Lead Healthcare Coordinator to join our team at Sunset Health. As a key member of our healthcare staff, you will play a vital role in ensuring the smooth operation of our medical services.

Job Description

The Lead Healthcare Coordinator will be responsible for coordinating patient care activities, managing daily medical assistant assignments, and collaborating with healthcare providers to deliver high-quality patient care. Key responsibilities include:

  • Coordinating staffing and scheduling to ensure efficient use of resources
  • Collaborating with care teams to assess patient needs and develop effective treatment plans
  • Maintaining accurate medical records and documenting patient information
  • Communicating with patients, families, and healthcare providers to ensure seamless care transitions
  • Providing education and support to patients and their families on health-related topics
Requirements and Qualifications

To be successful in this role, you must possess:

  • A degree or diploma as a Registered Nurse from an accredited school of nursing
  • Current Arizona or compact state RN licensure
  • Active CPR certification
  • At least three years of experience as a registered nurse in primary care ambulatory medicine or a related field
  • Bilingual proficiency in English and Spanish
  • Excellent communication and interpersonal skills
  • Able to work effectively in a fast-paced environment and prioritize tasks efficiently
Salary and Benefits

This is a full-time position with a competitive salary range of $75,000 - $90,000 per year, depending on experience. We also offer a comprehensive benefits package, including medical, dental, and vision insurance, 401(k) matching, and paid time off.
