We are seeking a dedicated and compassionate Registered Nurse to join our team as a Patient Care Coordinator for our Hemodialysis Unit. Job Responsibilities:
  • Deliver exceptional patient care by assessing, planning, and implementing individualized treatment plans.
  • Collaborate with the interdisciplinary team to ensure seamless transitions between care settings.
  • Provide ongoing education and support to patients and their families, empowering them to take an active role in their care.
  • Maintain accurate and detailed records of patient information, ensuring compliance with regulatory requirements.
  • Develop and implement quality improvement initiatives to enhance patient outcomes and satisfaction.
Requirements:
  • Degree in Nursing (BSN) from an accredited institution.
  • Current RN licensure in good standing.
  • Minimum 9 months of experience in nephrology nursing or related field.
  • Ability to lift up to 30 lbs and stand for extended periods.
Compensation and Benefits:

The salary for this position is estimated at $65,000 - $80,000 per annum, depending on location and experience. We also offer a comprehensive benefits package, including medical, dental, and vision coverage, as well as a 401(k) retirement plan.
