Physical Therapist

A highly skilled Physical Therapist is sought to join JAG Physical Therapy's team of dedicated healthcare professionals. As a key member of our interdisciplinary team, you will be responsible for assessing patients' strengths and weaknesses, developing and implementing personalized treatment plans aimed at improving their rehabilitation outcomes.
About the Role:
- You will engage in assessing patients' physical abilities and limitations, identifying areas for improvement and creating tailored treatment plans to address these needs.
- As a member of our team, you will collaborate with other healthcare professionals to provide comprehensive care, facilitate patient education, and monitor progress towards rehabilitation goals.
- You will be expected to maintain a positive, professional demeanor at all times, upholding the highest standards of patient confidentiality and discretion.
- Collecting and recording patient care data, including initial evaluations, daily notes, reassessments/progress reports, and discharge plans.
- Selecting and administering tests and measures to inform treatment decisions.
- Synthesizing evaluation results to formulate diagnoses, goals, prognosis, and plans of care.
- Establishing treatment plans based on evaluation results, goals, and best practices, collaborating with interdisciplinary teams, patients, and patient support systems.
- Providing education/training to patients and/or patient support systems on ADLs/IADLs, therapeutic activities, rehabilitation equipment prescription and training, and community reintegration.
- Overseeing support staff, including PTA, ATC, and PT aides.

- Licensed Physical Therapist in the state practicing in
- Completed state-regulated CEU requirements in appropriate states (NY, NJ)
- Valid CPR license
- Experience/knowledge with EMR software preferred
- Excellent oral, written communication skills
- Strong clinical decision-making skills
