Mass General Brigham is seeking a skilled Clinical Care Specialist to join our team in Milton, Massachusetts. This rewarding opportunity will allow you to provide quality, clinical services to patients in various settings in the community.

About the Role

This position involves delivering patient care within our organization's philosophy, policy, and standards of community health practice. You will be responsible for performing the admissions process and case management responsibilities according to MGB Healthcare at Home policies and standards of practice. Utilizing thorough and timely electronic documentation and interdisciplinary communications, you will ensure high-quality customer service and financial awareness.

Responsibilities
  • Assess, teach, treat, document, and coordinate skilled nursing care
  • Provide care coordination demonstrating high-quality customer service and financial awareness
  • Perform admissions process and case management responsibilities in accordance with MGB Healthcare at Home policies and standards of practice
Requirements
  • Graduate of an approved School of Nursing
  • Current Massachusetts license as Registered Nurse
  • Minimum of one year previous nursing experience in acute care setting; prior home care experience preferred
  • Able to work with various computer software and knowledge of hand-held patient care computer devices preferred
  • Strong interpersonal and customer service skills and ability to work with diverse population
  • Ability to communicate effectively in writing, verbally, and electronically
  • Ability to work as a member of an interdisciplinary team of healthcare providers
  • Ability to organize and prioritize work and adapt to changing situations
