Skagit Regional Health seeks a skilled and empathetic Nurse to join our team in providing exceptional hospice care to patients and their loved ones.

**Job Summary:**

  • As a compassionate hospice nurse, you will provide personalized support and care to patients on evening and night shifts, ensuring their physical, emotional, and spiritual needs are met.

**Key Responsibilities:

  • Assess patient needs and develop individualized care plans to address their unique circumstances.
  • Deliver high-quality care through phone and visit support, working closely with the interdisciplinary team to coordinate services and resources.
  • Provide education and instruction to patients, families, and caregivers, empowering them to take an active role in care management.
  • Maintain accurate and comprehensive patient records, documenting changes and progress.

**Requirements:

  • A current license to practice as a Registered Nurse in the state of Washington.
  • At least three years of experience in hospice, home health, medical/surgical, or skilled nursing facility settings.
  • Strong communication and interpersonal skills, with the ability to build trust and rapport with patients and their families.

**What We Offer:

  • A competitive salary range of $65,000 - $90,000 per year, depending on experience.
