Job Summary

We are seeking a Licensed Practical Nurse (LPN) to join our team of skilled home healthcare providers. As an LPN with Pinnacle Home Care, you will have the opportunity to work with a diverse range of clients, providing skilled nursing care under the direct supervision of a Registered Nurse.

Responsibilities
  • Carry out the plan of care under the supervision of the registered nurse.
  • Assist the Registered Nurse in performing specialized procedures as directed by the Registered Nurse under physician's orders.
  • Assist with the preparation, implementation, and continuing evaluation of the client care plan.
  • Recognize and understand the effects of social and economic problems upon clients and provide for the emotional and physical comfort and safety of clients.
  • Help teach the patient the appropriate self-care techniques.
  • Foster cooperative effort among personnel by understanding the functions of other persons involved in client care and by active participation in team and staff conferences.
  • Assume responsibility for personal and vocational growth and development through active participation in nursing organizations and participating in in-service programs and other on the job learning.
  • Prepare clinical and progress notes (not to include admission).
  • Observe, record, and report to the appropriate person patient symptoms, reactions, and changes.
  • Facilitate relationships between the clients and staff personnel and between clients and their families and visitors.
  • Observe, record, and report to the appropriate person the general physical and mental condition of clients and signs and symptoms which may be indicative of changes in condition.
  • Assist with the rehabilitation of clients according to the medical care plan.
  • Further assist in rehabilitation by reporting any abnormality in the client's range of motion, body mechanics, or body alignment.
  • Actively participate in nursing organizations and promote and participate in in-service programs.
  • Awareness of the Agency's QAPI program.
  • Active participation as required per policies and procedures related to the Agency's QAPI program.
  • Management and follow-up of customer complaints, following Agency's complaint policy.
  • Awareness of patient rights and the Agency's responsibility in honoring patient's rights.
  • Awareness of appropriate reasons for a patient discharge and Agency's policy on appropriate reasons for discharge.
  • Awareness of potential signs of abuse and neglect and the steps required to report suspected abuse and neglect.
  • Awareness and ability to design and identify patient-centered and individualized goals with realistic measurable timeframes under the supervision of a Registered Nurse.
  • Awareness of infection prevention and control techniques, both within their scope of practice and within the Agency's policy and procedures.
  • Awareness and participation in the Agency's infection control program.
  • Nursing bag technique policy acknowledgement and demonstrated competence.
  • Utilization of CRT, calculator, multi-line telephone, copy machine, basic medical equipment.
Requirements

To be successful in this role, you will need:

  • The ability to communicate well, both verbally and in writing.
  • The ability to access client's homes which may not be routinely wheelchair accessible.
  • Hearing, eyesight, and physical dexterity sufficient to perform a physical assessment of the client's condition and to perform client care.
  • Physical activities will include, walking, sitting, stooping, and standing and minimal to maximum lifting of clients and the turning of clients.
  • Graduation from an accredited School of Practical Nursing.
  • Must be licensed in the state of License Practical Nurse.
  • One or more years in an acute/sub-acute care facility. Home health experience preferred.
