Mass General Brigham is seeking a skilled Social Work Case Manager for a per diem position in New Hampshire.

As a Social Work Case Manager, you will be responsible for providing clinical social worker services to inpatients, outpatients, family, staff, and community members. Your duties will include:

  • Addressing the impact of diagnosis on patients and families
  • Conducting patient and family counseling to enhance their ability to cope effectively
  • Providing education and support to community groups and/or staff
  • Initiating advance care planning with patients and families
  • Supervising social work interns doing field placements
  • Assessing and referring patients for DCYF and BEAS
  • Assessing and referring patients for cases of domestic violence
  • Providing counseling and support to birth mothers in adoption cases
  • Responding to traumatic events and providing crisis intervention
  • Providing grief support and counseling as needed
  • Assessing and supporting patients receiving palliative care
  • Providing ongoing intervention and support to patients and families in the outpatient setting

Requirements:

  • Master's degree in Social Work from an accredited college or university
  • One year of experience in a healthcare setting
  • Knowledge of medical terminology
  • Ability to organize time and establish priorities
  • Ability to communicate in written, electronic, and verbal formats
  • Ability to work in a stressful environment and handle sensitive issues
  • Ability to maintain confidentiality
  • Ability to operate a computer
