Helping Hands Home Care is a reputable organization seeking a compassionate and skilled Registered Nurse to join our Hospice Division as an End-of-Life Care Specialist.

Estimated Salary: $85,000 - $115,000 per year

About the Role:

We are committed to providing exceptional care to patients and their families during their most vulnerable time. As an End-of-Life Care Specialist, you will be part of an interdisciplinary team working together to deliver patient-centered care. Our ideal candidate possesses strong communication skills, empathy, and a passion for palliative care.

Key Responsibilities:

  • Symptom Management: Collaborate with the patient's attending physician and Medical Director to ensure effective symptom management through thorough assessments.
  • Plan of Care: Evaluate treatment plans and medical conditions according to individualized care needs, adjusting as necessary.
  • Continuity of Care: Manage interdisciplinary care while promoting continuity of care with admissions, transfers, and discharges.
  • Families Educator/Advocate: Counsel patients and families on disease progression, incorporating them in plan development and active listening.
  • Documentation: Complete clinical documentation following agency protocol and Medicare/Federal guidelines.
  • Operations: Supervise total patient care provided by nurse aides and LPNs, attending in-service training and mandatory meetings.

Requirements:

  • Exceptional communication and positive attitude.
  • Patient-focused with attention to detail, observation, and adherence to protocols.

Education and Experience:

  • Graduate of an accredited registered nursing program.
  • Bachelor's degree in Nursing preferred.
  • Certified Hospice and Palliative Nurse (CHPN) certification preferred.
  • Minimum 2-4 years RN experience in hospice or home health agencies or related programs.

Licensure and Certifications:

  • Current Registered Nurse license in good standing in the State of Pennsylvania.
  • Valid driver's license and reliable transportation.
