Weekend Community Health Nurse

The RN will be responsible for assessing, planning, implementing, coordinating, and evaluating the strategies and services required to meet the patients' end-of-life care needs.
Utilizing the nursing process, relationship-centered care principles, and a holistic philosophy, the RN will create and continually adjust an outcome-based plan of care to ensure the changing needs of the patient are met.
Required qualifications include an Associate's Degree in Nursing and 1 year of relevant experience, preferably in hospice or a related field.
Preferred qualifications include a Bachelor's Degree in Nursing.
