We are seeking a compassionate Nurse Practitioner (NP) to join our team at Abode Care Partners. As a Medical Care Provider, you will play a vital role in providing evidence-based medical care to seniors and patients with complex needs.

Responsibilities- Evidence-Based Medical Care: Plan, direct, and evaluate patient medical needs, ensuring high-quality care that meets individual requirements.
- Patient Examination and Treatment: Examine and treat chronic and urgent episodic illnesses, minor injuries, accidents, and other injuries or illnesses, using your expertise as a Nurse Practitioner.
- Lab Testing and Collaboration: Order and interpret laboratory and diagnostic testing as needed, working collaboratively with caregivers and healthcare professionals to provide interdisciplinary care and maintain comprehensive plans of care.
- Patient Referrals and Record-Keeping: Provide referrals to other healthcare practitioners when necessary and ensure accurate, up-to-date patient medical records are maintained.
- Nurse Practitioner Licensure: Active state NP license and current national board certification required.
- Clinical Experience: Five or more years of nursing experience; at least two years of NP experience preferred.
- Communication Skills: Strong interpersonal communication skills – verbal and written – essential for effective collaboration and patient care.
- Technical Competencies: Proficiency in medical instruments and equipment, as well as computer-based data management programs, electronic medical record (EMR) systems, and Microsoft Office applications.
- Competitive Compensation: Salary range: $100,000 - $130,000 per year.
