About the Job

As a Care Coordinator for Palliative Care Services, you will play a vital role in addressing the physical, mental, social, and spiritual needs of patients with life-limiting illnesses. Working collaboratively with interdisciplinary teams, you will assess patients' goals, needs, and strengths to develop comprehensive care plans that promote their well-being.

Responsibilities

  • Assess patients' physical, emotional, and social needs to identify areas requiring support
  • Develop and implement individualized care plans that address patients' goals, needs, and strengths
  • Collaborate with healthcare staff, patients, and family members to provide education and support on palliative care services
  • Link patients with community resources to ensure seamless transitions between care settings
  • Participate in quality improvement initiatives to enhance patient care and outcomes

Requirements

  • Masters degree in Social Work
  • Three years of direct patient care experience
  • Preferred: State of CT Licensed Clinical Social Worker (LCSW)
  • Preferred: Experience in internal medicine, geriatrics, or oncology
