Job Title: ECM Care Coordinator

Clinica Sierra Vista is a leading healthcare provider seeking a highly skilled ECM Care Coordinator to join our team. As a key member of our healthcare team, you will play a vital role in ensuring seamless patient care and coordination.

Job Summary:

The ECM Care Coordinator will be responsible for coordinating patient care, ensuring timely and effective communication between healthcare providers, and promoting patient engagement and empowerment. This role requires strong organizational and communication skills, as well as the ability to work effectively in a fast-paced environment.

Key Responsibilities:
  • Meet with new patients to explain PCP, Patient Portal, and care access
  • Assign patients to provider panels, ensuring balance and continuity
  • Review monthly panel reports and adjust PCP assignments as needed
  • Collaborate with healthcare providers to ensure continuity of care
  • Proactively engage priority patients to promote expanded access clinic utilization and reduce unnecessary ER visits
  • Run and analyze standard CSV reports to inform care coordination decisions
  • Oversee and analyze data from assigned panels to identify trends and areas for improvement
  • Ensure clinic-wide compliance with CSV, PCMH, CMS, and CDPH requirements
  • Meet or show consistent improvement on CSV clinical quality goals
Requirements:
  • Medical Assistant certification or program completion required
  • Strong computer proficiency, including Excel, Word, Outlook, and Electronic Health Records
  • Bilingual (Spanish-English) preferred
  • Excellent internal and external customer service skills
  • Ability to maintain confidentiality and handle sensitive information
  • Strong organizational and time management skills
  • Ability to work independently and as part of a team
  • Highly organized and detail-oriented
  • Knowledge of modern office equipment, systems, and procedures
  • Ability to multitask and work efficiently in a potentially stressful environment
