Job Summary
We are looking for a compassionate and dedicated High Acuity Nurse to work with us in providing customized care plans that meet the unique needs of our patients. As a member of our team, you will have the opportunity to work in a dynamic and supportive environment, collaborating with other healthcare professionals to deliver top-notch patient care.
Key Responsibilities
- Assess patient needs and develop individualized care plans to ensure optimal outcomes.
- Provide high-quality nursing care to patients in their homes, including medication management, wound care, and vital sign monitoring.
- Collaborate with other healthcare professionals to ensure seamless transitions of care.
- Maintain accurate and detailed records of patient progress and treatment plans.
What We Offer
We offer a competitive salary of $$60,000 - $80,000 per year, depending on experience, as well as a comprehensive benefits package, including medical, dental, and vision insurance, 401(k) matching, and paid time off.
Requirements
- Curently licensed as a registered nurse (RN) in the state of employment.
- 1+ year of experience as a registered nurse in an acute care setting.
