About the Role

We are seeking a highly skilled Patient Care Coordinator to join our team at Providence Service. As a key member of our clinical team, you will be responsible for coordinating seamless care for patients in a surgical and/or procedural clinic setting.

This is a challenging and rewarding role that requires strong organizational and communication skills, as well as the ability to work effectively in a fast-paced environment. You will be responsible for balancing a complex daily workload, managing multiple priorities, and providing exceptional customer service to patients and their families.

Key Responsibilities
  • Coordinate patient care and services to ensure seamless transitions and optimal outcomes.
  • Develop and maintain strong relationships with patients, families, and healthcare providers to ensure effective communication and collaboration.
  • Manage and prioritize multiple tasks and responsibilities to meet deadlines and achieve goals.
  • Provide exceptional customer service and support to patients and their families, addressing their needs and concerns in a timely and professional manner.
Requirements
  • 2 years of experience in a healthcare setting with increased responsibilities or customer service type role.
  • Electronic Medical Records (EMR) experience is a plus.
