Job Summary

We are seeking a skilled Care Management Professional to join our team at the University of Maryland Medical System. As a key member of our Community Health team, you will play a vital role in identifying and addressing gaps in patient care, ensuring seamless transitions across the continuum of care.

Key Responsibilities:

  • Collaborate with the RN Care Manager and Interdisciplinary Care Team to address post-discharge and post-acute care needs, coordinate referrals, and address social determinants of health.
  • Provide administrative support to an assigned organizational unit, performing tasks under moderate supervision.
  • Document patient medical records and maintain confidentiality of protected health information.

Requirements:

  • Bachelor's degree in a healthcare-related field or equivalent experience.
  • Minimum two years' experience in care management, coaching, or community health work.
  • Valid driver's license and reliable transportation (may be required for offsite visits).

Competitive Compensation:

The estimated salary for this position is $26.40-$28.33 per hour, based on experience and qualifications. Our comprehensive benefits package includes health, dental, and vision insurance, retirement savings plan, and paid time off.
