Travel Nurse RN Care Manager

We are seeking a skilled and compassionate Hospice Care Manager to join our team. This is a fantastic opportunity for experienced nurses to make a meaningful difference in the lives of patients and their families.
Job OverviewAs a Hospice Care Manager with Healthcare Support, you will be responsible for delivering high-quality patient care and providing emotional support to patients and their loved ones. You will work closely with a multidisciplinary team to develop and implement individualized care plans that address the unique needs of each patient.
Key Responsibilities- Provide expert hospice care to patients and their families, ensuring dignity, comfort, and quality of life.
- Collaborate with healthcare professionals to develop and implement comprehensive care plans that meet the unique needs of each patient.
- Conduct regular assessments to identify patient needs and preferences, and make recommendations for care plan adjustments as needed.
- Maintain accurate and detailed records of patient interactions, including progress notes, treatment plans, and discharge instructions.
To be successful in this role, you will need:
- A valid RN license and 1 year of experience in hospice care.
- Current BLS certification from the American Heart Association.
- Excellent communication and interpersonal skills, with the ability to build strong relationships with patients, families, and healthcare professionals.
- The ability to work independently and as part of a team, with a high level of flexibility and adaptability.
We offer a competitive salary package, including:
- A weekly stipend of $1393, making your total compensation $2510.2 per week.
- A local hourly rate of $56.19 to $57.19 an hour, depending on your experience and qualifications.
- Up to 5 shifts per week, with a guaranteed 40 hours of work per week.
