Rehabilitation Nurse Job Description

We are seeking a skilled Rehabilitation Nurse to join our team at Swedish Cherry Hill. As a Rehabilitation Nurse, you will play a vital role in providing high-quality patient care and contributing to the success of our rehabilitation unit.

Key Responsibilities:
  • Assess, identify, and prioritize patient needs, developing and implementing individualized care plans to achieve optimal outcomes.
  • Collaborate with interdisciplinary teams to coordinate patient care, ensuring seamless transitions and effective communication.
  • Provide education and support to patients and families, promoting independence and self-management skills.
  • Participate in quality improvement initiatives, contributing to the development and implementation of evidence-based practices.
  • Document patient progress, maintaining accurate and comprehensive records.
Requirements:
  • Graduation from an accredited nursing program.
  • Current Washington State Registered Nurse License.
  • Basic Life Support (BLS) certification.
  • 1 year of rehabilitation nursing experience.
Preferred Qualifications:
  • National Certification in Rehabilitation Nursing.
