Job Summary

We are seeking a skilled Registered Nurse to join our team at Cheshire Medical Center. The successful candidate will have a strong background in nursing, excellent communication skills, and the ability to work effectively in a fast-paced environment.

About the Role

The Clinical Care Specialist will be responsible for providing high-quality patient care under the supervision of the Nursing Team Leader, Clinical Leader, and Department Director. This includes assessing patient needs, developing individualized care plans, and implementing evidence-based interventions to promote optimal health outcomes.

Key Responsibilities
  • Provide direct patient care, including assessments, interventions, and education
  • Develop and implement individualized care plans to meet patient needs
  • Collaborate with interdisciplinary teams to coordinate patient care
  • Document relevant data and care processes
Requirements

To be successful in this role, you will need:

  • A current RN license in the State of New Hampshire
  • Minimum one year of nursing experience preferred
  • Certification in BCLS required
  • Excellent assessment, communication, interpersonal, and organizational/time management skills
