Hospice Registered Nurse Job Description

About the Role:

We are seeking a compassionate and skilled Hospice Registered Nurse to join our interdisciplinary team at Healthkeeperz, Inc. As a key member of our team, you will play a vital role in providing high-quality care to our clients and their families.

Key Responsibilities:

  • Assess clients' physical, emotional, and psychosocial needs and develop individualized care plans
  • Coordinate care with the interdisciplinary team, including social workers, chaplains, and volunteers
  • Provide skilled nursing care and education to clients and their families
  • Advocate for clients and families, ensuring their needs are met and their voices are heard
  • Participate in bereavement support and provide emotional support to clients and families
  • Document and manage medications, DME, and supplies
  • Participate in weekly meetings and IDG meetings to ensure seamless care coordination

Requirements:

  • Active NC nursing license
  • At least one year of hospice or home health nursing experience
  • Ability to speak in front of groups and maintain high-quality customer service
  • Working knowledge of computer programs and ability to learn new systems

Physical Demands:
