Senior Medical Professional for Abode Care Partners

We are seeking a skilled Nurse Practitioner (NP) to join our team at Abode Care Partners. The successful candidate will provide compassionate medical care to seniors and patients with complex needs in various settings.
Responsibilities:- Perform evidence-based medical care by planning, directing, and evaluating patient medical needs.
- Examine and treat chronic and urgent episodic illnesses, minor injuries, accidents, and other injuries or illnesses.
- Order and interpret laboratory and diagnostic testing as needed.
- Collaborate with caregivers and other healthcare professionals in providing interdisciplinary care and maintaining a comprehensive plan of care.
- Provide referrals to other healthcare practitioners when necessary.
- Evaluate and update patient medical records.
- Active state NP license.
- Current NP National Board Certification.
- Active state DEA licensure.
- Master's degree in Nursing (MSN).
- Five or more years of nursing experience; at least two years of nurse practitioner experience preferred.
