Job Description

We are currently seeking a Full-Time Residential Coordinator for the Supported Living Program. As a Residential Coordinator, you will serve as a team leader who coordinates and provides supports and advocacy services for adults with developmental disabilities and significant medical or mental health issues, who live independently in their own homes.

Key Responsibilities
  • Create and implement action plans that foster the health, safety, and individual goals of our clients
  • Provide supports and advocacy services for adults with developmental disabilities
  • Collaborate with clients, families, and healthcare providers to develop and implement care plans
Requirements
  • Minimum two years working with adults with intellectual and/or developmental disabilities with significant medical or mental health issues
  • Minimum one year performing lead duties, including creating action plans and ISP implementation
  • Familiarity with SSI, Medicare, and Representative Payee services and Benefits Coordination
  • A valid driver's license and an operable and safe personal vehicle with liability insurance
Benefits

This is a full-time position with expected work weeks of 40 hours. Occasional paid overtime may be required. This is a salaried, non-exempt position eligible for bonus and annual increases. The estimated salary for this position is $45,000 - $55,000 per year, depending on experience and qualifications.
