Job Summary

We are seeking a dedicated and compassionate Licensed Practical Nurse to join our team at Consulate Health Care. As an LPN, you will play a crucial role in providing high-quality nursing care to our residents, collaborating with our healthcare team to implement individualized treatment plans, and supervising and evaluating CNAs to ensure they deliver care following our high standards.

Responsibilities
  • Provide high-quality nursing care to residents, including administering medications, wound care, and vital sign monitoring
  • Collaborate with the healthcare team to develop and implement individualized treatment plans for each resident
  • Supervise and evaluate CNAs to ensure they are delivering care according to our standards
  • Oversee fall-risk and pressure ulcer prevention programs, actively working to prevent and manage these conditions
  • Conduct thorough evaluations of each resident's medical status upon admission and throughout their course of treatment, documenting any changes and adjusting care plans as necessary
Requirements
  • Active license to practice as an LPN
  • Experience in long-term care, skilled nursing, or rehabilitation facilities is preferred
