Delaware Community Care Coordinator

We are seeking a highly skilled Community Care Coordinator to join our team at CAI in Delaware. This is an exciting opportunity to make a meaningful difference in the lives of our clients and their families.
About the Job
The successful candidate will be responsible for coordinating community-based care for members with chronic and complex healthcare needs. This role involves conducting face-to-face assessments, developing care plans, and facilitating transitions to appropriate care settings, all while minimizing costs and promoting optimal outcomes.
Key Responsibilities
- Assessments and Care Planning: Travel to members' homes, nursing facilities, and other community-based settings to complete face-to-face needs assessments with subsequent telephonic contact with the member in accordance with state and national guidelines, policies, procedures, and protocols.
- Care Coordination: Assess, plan, coordinate, implement, and evaluate care for eligible members with chronic and complex health care, social service, and custodial needs in a nursing facility or home and community-based care setting.
- Transition Management: Facilitate authorization, coordination, continuity, and appropriateness of care and services in community or HCBS settings.
- Member Education: Educate members or caregivers regarding healthcare needs, available benefits, resources, and services, including available options for long-term care community or facility-based service delivery.
Requirements
- Qualifications: Current Registered Nurse license in the state of Delaware, 5+ years of Case Management and Discharge Planning experience, including experience discharging members from a facility setting.
- Skills: Experience completing assessments, developing service plans, and care plans; collaborating with PCPs, Occupational Therapists, Behavioral Health, and Providers; ordering DME equipment; educating and providing resources for members' social determinants.
- Physical Demands: Ability to safely and successfully perform the essential job functions consistent with the ADA and other federal, state, and local standards.
Compensation
The estimated salary for this role is $65,000 - $85,000 per annum, depending on experience. We offer a comprehensive benefits package, including health insurance, retirement savings, and paid time off.
